( Status ) I still own this costume.

( Construction Notes ) I had originally planned to make this costume last year for Anime Expo 2005 and the World Cosplay Summit, but it sadly was delayed. This year though, I was finally able to make it and wear it for the first time to Fanime. The uniform skirt is made of blue polyester poplin and a plain white cotton dress shirt. The patch was an image I found in a Japanese cosplay magazine which I scanned into the computer and redrew using photo shop. I then printed the image onto fabric printer paper and edged it with a zigzag stitch to resemble a real patch. Lastly, the helmet is constructed of foam core board, foamy, lots of hot glue, spackling compound, and many layers of modge podge, spray paint and varnish. Don't ask for a step by step process, it would take too long. The only thing missing are the indented holes on the sides of the helmet. I may or may not add those at a later date using acrylic paint to imply shadows.
